INSURANCE BILLING

I am always happy to provide a receipt of payments for you to submit to your insurance carrier. These will include a “procedure code” ie. Individual session 60 minutes, a “diagnosis code”, the problem we are trying to resolve, date of service, and payment received. I am currently able to directly bill the following insurance companies:

• MHN/HealthNet including Tricare West
• First Choice Health PPO
• Regence Blue Shield
• Aetna including GEHA
• United Health/Optum
• Primera Blue Cross including LifeWise
• Apple Health Plans including:
  • Community Health Plan of Washington(CHPW)
  • Molina Health Care
  • Co-ordinated Care

Please have your insurance company name and your insurance ID number or member number available when you call to make an appointment. Most companies require pre-authorization for non-emergency treatment.

Please remember that billing insurance requires permission to give the company access to my records of your treatment. We will discuss your diagnosis code prior to submission as this can affect your life well beyond the duration of our work together.
